Visit District Health Management Teams (DHMTs) or local hospitals.

How to apply for service

• Visit nearest health facility to fill Form MH 3091.
• Submit X-Rays, mining history, laboratory and pathology results to Environmental and Occupational Health Division, Ministry of Health Headquarters, ground Floor, Office no GF9 or GF10.
