•  Request letter for the approval of the qualification and experience of a person (incumbent) intended to be appointed as manager of a mine, quarry or works in terms of regulation 6(1) of the Mines, Quarries, Works and Machinery Regulations, Chapter 44:02
• Curriculum vitae of the person to be appointed as manager, certified copies of his certificate and proof of work experience
