Service Description

To assess and determine claims for Workers Compensation purposes.

 How do I apply for the service?

Visit the  nearest Labour Offic and complete  the following forms;  

•  Form BL 43/10-Injury and Disease report form to be completed by the employer within 17 days.
•  Form BL 43/02-Wages of worker to be completed by employer
• Form BL 43/03- Medical Examination form to be completed by Medical Doctor.
• Form BL 43/07-Certificate of insurance to be completed by insurance company.
• Form BL 43/11-Notification of time and place of attendance by Medical Practitioner to be completed by employer.

Additional information

• Covering letter from the employer confirming injury
• Claimant’s statement of injury.
• 2 witnesses statements.

In case of Road Traffic Accident (RTA)

• Police report.
• Claimant’s sworn statement
•  2 witnesses sworn statements.
•  

In case of Death

• Post Mortem report

• Death certificate

How much does it cost?

• The service is offered free of charge.
