From the Financial Statements, Tables and Estimate of Consolidated and Development Fund Revenues book compiled by the Division as well as the Ministry Website

3. How does one apply for the service?  

One does not need to apply for the service. Instead come to the office, buy the book from Government Printers or Ministry Library

4. Costs involved on obtaining service

Currently the tables book is P 30.00 at the Government Printers
