HOW DO I OBTAIN THE SERVICE?
By filling the application form at the licensing office .
HOW DO I APPLY FOR THE SERVICE?
- You have to fill in the application form at the licensing office.
- The requirements for applying for the licenses are as follows:
- Certificate of incorporation, form 2, form 4, share certificates and business name or certified copy of (Omang) National Identity, and certified work permits and residence permits for non citizens.
- Proof of availability of business premises (written undertaking lease in the agreement of the deed).
- Satisfactory Physical Planner’s report for zoning purposes.
- Satisfactory Environmental Health report of the premises.
- Finally you remit the sum of P100.00 application fee at Council Revenue Office when submitting the application forms
COSTS INVOLVED
The application fee for every licence is P100 and annual licence fee varies from licence to licence as indicated below:
