SERVICE DESCRIPTION
It is the authorization of a dead body to be conveyance outside Botswana.
HOW DO I OBTAIN THE SERVICE
- Provide death certificate (Civil and National Registration office).
- Provide certificate of precaution (Hospital)
Who qualifies for the service?
- Any dead body to be conveyance outside Botswana
HOW DO I APPLY FOR THE SERVICE?
- Application by relatives/ next of kin to the deceased
- Provide circumstances under which it is required to remove the body
- Proposed mode to conveyance and disposal of the body
- Precautions taken to prevent danger to health or any cause or offence arising
- Duly certified copy of certificate of cause of death signed by a qualified medical practitioner.
- Cover letter
