Physical presentation to office 93 or 97

HOW DO I APPLY FOR THE SERVICE?

Collect and complete application forms(attached) and attach the following;

(a)   Certified copy of Omang

(b)   Two passport sized photos (black and white)

COST INVOLVED IN OBTAINING THE SERVICE?

The certificate is paid P100 upon collection
