Re-issuing a license which might have been lost, destroyed or mutilated.

HOW DO I OBTAIN THE SERVICE?

Physical presentation to office 93 or 97

HOW DO I APPLY FOR THE SERVICE?

Collect and complete a duplication form and attach the following;

(a)   Copy of a police report

COST INVOLVED IN OBTAINING THE SERVICE?

Upon submission of the application for duplicate you pay P100 for liquor or trade and P50 for manufacturing application.

Duplicate license fee is P100 for Trade, liquor and manufacturing
